*Required for anyone who participates in off-site trips or events that require riding the church van.
*Required for nursery volunteers.
*Calvary Background Checks are required to drive, teach, OR chaperone any activity or event with minors.
All background checks must be renewed every 3 years.
Please bring:
- your government issued ID or driver's license OR provide a color copy of BOTH sides of your ID.

If submitting a Driver Background Check:
-In addition to the Driver Background Check Form, you must have passed a Calvary Background Check within the last 3 years.
*Must be age 21–75 to drive the church van. Ages 76+ are required by our insurance company to provide a doctor's release statement.
